Place and pay for your order online and you can collect what you ordered from Form, Valley Road, Msida, Malta.

You will be notified when your order is ready for collection: for in-stock items, this is within 3 days from the date you place the order, whilst the items that are not in stock will be available within 5 days of arrival at our warehouse.

Kindly bring your confirmation e-mail with you as proof of purchase.

Parcel Delivery

Parcel delivery is available for most items such as home accessories, gifts, lighting and small furniture. This service will be provided by professional couriers at the following charges:

Small items: €6

Medium: €12

Large but light items: €25

For orders, whether the item is one or more, having a total value of three hundred euros, the delivery charge is waived.

In case of multiple items, the order is to be collected or delivered when all items are available at our stores.

*** Standard delivery does not include assembly or unpacking.  On some occasions, the courier may require assistance unloading and carrying larger items.

Small furniture delivery

Our small furniture delivery service offers delivery right to your front door. If the destination is an apartment, flat or complex, the order will be delivered inside the entrance of the building for €25 if the item in question can be carried by one person. Should 2 persons be required for the job, the charge shall be €40. However, if the value of the order is €300 or more, no charge shall be levied.

This service shall be rendered by a professional courier  or the company’s own personnel. Unpacking and assembly are not included in the service.

Large furniture delivery (appliacable to On Line and In Store purchases)

This service shall be provided by professionally trained personnel engaged by Form.

Delivery, installation and collection of waste packaging are included in the price.

The customer shall bear all charges and expenses for cranes/lifters, local council permits and warden service when required. Form will assist the customer to procure the services of the lifter/crane on behalf of the customer but the customer shall be responsible to obtain all permits that are required for the job.

In view of the inherent danger in the uplift of goods by mechanical means, the customer accepts to exempt Form from responsibility for this aspect of delivery. The legal responsibility for this aspect of carriage shall be between the customer and the owner of the lifter/ crane. Any damage to any person/s  as  well as any property or goods to  whomsoever they belong that may result from the carriage by lifter / crane and the goods being lifted shall be at the risk and responsibility of the customer even though Form  personnel shall be actively assisting in the delivery operation in the loading and offloading  of the goods being carried.

The customer shall ensure that all items in the premises of the customer that are breakable or can otherwise suffer damage during the carriage of goods at the given address are removed beforehand to provide proper unrestricted access throughout. No responsibility shall attach to Form for any damage to floors, walls and bannisters that may be damaged during delivery and installation of furniture at customer’s premises.


Goods that have been made to order in a specific finish or upholstery colour as per customer’s specifications cannot be returned. Stock items can be returned provided that we are notified within 48 hours of receipt of your order (by e-mail to: [email protected] ) and provided the item is returned within 3 days from delivery. The product you return must be in new, unused, condition with all its original packaging and product tags still attached. New and unused shall mean there are no marks on the items or signs of wear and use. The item shall be returned at the charge of the customer. We can collect at a charge to be agreed with our Customer Care personnel.

If your order is received damaged or defective, we will endeavour to repair it or a replacement will be manufactured and delivered as soon as possible. No claims whatsoever shall be entertained for a replacement for damaged or missing goods unless it is notified to the delivery personnel at the time of delivery. The manufacturer’s warranty on faulty goods remains valid for the period it is given. In case of damage or defect, the customer shall only be entitled to retain a maximum 10% value of the invoice till the defect is remedied or a replacement is supplied.

Once an order has been placed, it will not in most cases be possible to change the order whatever the reason (ex: mistake, change of mind about colour/ size) if it has entered into the production process. Naturally, we will use our best endeavours to intervene and try and assist you with your request but the ultimate decision rests with the manufacturer whose discretion is not subject to challenge.

Our personnel will unpack all heavy furniture and place it where you direct them. During installation of any furniture, you are kindly requested to allow our personnel to work at ease and unsupervised and to present you with the result of their work for your approval once the job has been finalized. You are kindly requested to provide a clean area with decent climate control at their place of work as well as necessary toilet and hand cleaning facilities.
